With the aim of informing, this section contains the forms that are most frequently used in the various phases of the Operative Unit's activity. They are in Word or RTF format files that can be easily examined and printed; they concern the so-called "Patient's steps".
1. The form used to book the hospitalisation period is the only document that grants access to the waiting lists for scheduled operations. It is filled in by the U.O.'s doctor in any one of the out-patients departments: divisional, free-lance or orthopaedic emergency room.
2. Information on the Pre-hospitalisation Service. This form contains information on how the Service works. It is given to the patients in the form of a brochure.
3. Receipt stating the enrolment of the patient in the waiting lists: this certification is signed by the Pre-hospitalisation staff in charge and given to the patient. It contains the date of enrolment in the booking register, the priority level assigned and the expected waiting period.
4. The Pre-hospitalisation form is filled in by the nursing staff of the Pre-hospitalisation Service; this form contains a very simple outline of all the procedures that are carried out in this phase.
5. Acceptance and confirmation of the waiting time: this statement is filled in by the patient to declare that he/she is well aware of the time he/she will have to wait and of the priority level he/she was assigned. Through this statement the patient expresses the will to undergo the programmed surgery in our medical structure. Moreover, this form is also used to sign a further agreement in the case of operations that require a longer waiting period (Prosthetic Surgery).
6. Waiting list revision: this form allows a periodic revision (every six months) of the waiting lists. This is necessary in order to keep the quantity and quality of the booking register for programmed hospitalisations under control.
7. Directions to be followed before hospitalisation: this list contains all the rules that patients need to follow in the period of time right before the actual hospitalisation. Since patients are hospitalised on the very day of the operation, such rules are considered to be very important.
8. Day Hospital examinations prescription: this is a non-obligatory prescription for simple examinations that are usually prescribed before any daytime cycle surgical treatment.
9. Directions for Day Hospital operations: these are a group of simple advice that should be followed before such operations.
10. Discharge from the hospitalDimissione: this form is given at the end of the hospitalisation period and it is the same for any kind of orthopaedic hospitalisation.
